Conversion of non-adjustable gastric band to Roux-en-Y gastric bypass assisted by robot

ABSTRACT
Introduction: The non-adjustable gastric band is a restriction technique described in 1978. In the short term is associated with a good response in terms of weight loss and low perioperative morbidity. In the long term, the recovery of lost weight and the incidence of complications are frequent. The withdrawal by laparoscopy and conversion to gastric bypass in Y de Roux or Gastric Sleeve is a viable alternative with good results in the face of failure and/or development of complications.
Case presentation: Two cases of non-adjustable gastric band removal and conversion to gastric bypass in Y de Roux by laparoscopic robot-assisted approach in patients diagnosed with obesity due to excess weight lost and the development of complications attributed to the presence of the gastric band. Firm adhesions were found in the periphery of the physical band, which made it difficult to remove them. There are no perioperative complications. The console time and the total surgical time was 196 and 231 minutes in patient 1, and in patient 2 was 204 and 238 respectively.
Conclusions: The non-adjustable gastric band has no indication at present time due to its low results and high long-term morbidity in relation to other bariatric surgery procedures. Its withdrawal hinders the conversion to Y de Roux gastric bypass because of the intense fibrosis it generates. The pulling force offered by the robotic approach represents an advantage over conventional laparoscopy, which facilitates the procedure.
