Intraosseous maxillary venous malformation: microvascular reconstruction (case report and literature review)

ABSTRACT

The maxilla and jaw are the most commonly affected bones of the facial skeleton, the presence of intraosseous malformations is rare, as they usually affect soft tissues. Venous malformations are the most common type of low-flow malformation. They are composed of vascular channels with stasis. Although present at birth, they may remain hidden until youth or old age. Treatment for venous malformations includes surgical excision, sclerotherapy, radiation therapy, electrocoagulation, cryotherapy, and magnesium-laser instillation. Depending on these treatments, size and location, as a first option maxillectomy is performed taking into account the classification of Cordeiro. Due to its exophytic clinical aspect, there are clinical differentials with immunohistochemistry that helps the definitive diagnosis. Thus, image studies such as computed tomography, magnetic resonance and angioresonance also serve as diagnostic aids, delimiting their extent for proper treatment. Therefore, we present the case of a 22-year-old female patient, with histopathological report with diagnosis of tumor with venous malformation of low maxillary flow, treatment and reconstruction with microvascular flap with fibula for later rehabilitation with dental prosthesis.
