Peritonitis by barium in an infant with intestinal invagination, a case report

ABSTRACT

Introduction: Non-surgical treatment of intussusception is effective in 90% of cases, during the first 24 hours. Intestinal perforations occur in 0.5-2.5% of attempts to reduce with barium or hydrostatic, and between 0.1 and 0.2% with air. Chemical peritonitis by barium reaches a mortality up to 50%. Barium absorbed in the peritoneal cavity produces serious and irreversible damage. Case presentation: Infant with intestinal intussusception; after barium enema free contrast medium in the cavity was detected. Laparotomy was performed, finding ileocecocolic invagination perforation, and barium adhered to intestinal loops. The patient developed septic shock and required four surgical interventions. Conclusions: Barium enema must not be used as a diagnosis and treatment, due to the high morbidity and mortality in pediatric patients.
