ABSTRACT

Objective: To determine the frequency and degree of fecal incontinence in patients subjected to partial lateral internal sphincterotomy as part of the surgical treatment for chronic anal fissure.
Setting: Coloproctology unit of the gastroenterology service of the General Hospital of Mexico.
Design: Retrospective, transversal, and descriptive study.
Statistics: Percentages as summary measure for qualitative variables.
Material and methods: The study period covered from July 2003 to June 2005, in patients with a diagnosis of chronic anal fissure subjected to partial lateral internal sphincterotomy. The direct variable was fecal incontinence assessed preoperatively by means of the Jorge and Wexner classification, and then at one and three months post-surgery. Secondary variables were: age, gender, time of disease evolution, location, used surgical treatment, time of cicatrization, post-surgical complications and their resolution.
Results: 160 patients were women and 68 were men. None presented preoperative incontinence: at 1 month after surgery, 38 patients (16.96%) presented some degree of fecal incontinence. At three months after surgery, incontinence decreased in frequency and degree: from 24 patients with a score of 1, 16 presented a score of 0 and 8 persisted with a score of 1. From 12 patients with a score of 2, 7 decreased to a score of 0, and 5 to a score of 1.
Conclusion: This study indicates that 6.5% of patients subjected to partial lateral internal sphincterotomy presented a minimal degree of fecal incontinence at 3 month post-surgery.
