Myxedema Coma. When to suspect it?

ABSTRACT

Myxedema Coma is the most severe clinical manifestation of an infra-substituted or undetected hypothyroidism, usually occurs in geriatric patients and is considered a clinical emergency because of its high mortality (40-50%). It worsens by precipitating factors such as infections, medications, acute myocardial infarction, and exposure to cold. Clinically, it does not manifest itself as a comatose state, but evolves through several stages of neurological deterioration, going through disorientation, obtundation, drowsiness, until reaching lethargy. A clinical case of a patient in her eighties, is presented, who went to the emergency room due to fluctuations in the alert state, long time of evolution, with data of hypometabolism. It is also discussed the use of a myxedema coma scale to determine the onset of intensive care and the need for amine support or advanced airway management.
