Diffuse large B-cell metastatic lymphoma to the testis and bladder. A case report

ABSTRACT

Clinical case description: A 44-year-old man was diagnosed with germinal center B-cell-like diffuse large B-cell lymphoma and metastasis to the right testis and bladder. He presented with edema and pain in the lower extremities, urinary retention, and acute kidney injury. A tomography scan upon admission revealed a retroperitoneal lymph node conglomerate, and serum tumor markers were altered. A Doppler testicular ultrasound was performed, despite a normal physical examination of the genitals, revealing a right testicular tumor. Radical orchiectomy was performed, along with endoscopic urinary diversion with double-J catheters, and neoplastic bladder involvement was observed. Histologic study of the testis and bladder reported a poorly differentiated malignant tumor of probable metastatic origin and immunohistochemical study stated germinal center B-cell-like diffuse large B-cell lymphoma.
Relevance: Non-Hodgkin lymphomas are the main causes of secondary lymphoma of the testis and bladder
Clinical implications: Secondary involvement of the testis and bladder is important to consider in young male patients with retroperitoneal masses.
Conclusion: Secondary involvement of the testis and bladder is unusual in this type of lymphoma. The present case was reported to demonstrate the importance of considering the diagnosis of said compromise in young patients with retroperitoneal masses.
