ABSTRACT

Introduction. During pregnancy the urachus represents the connection between bladder and alantoides therefore urachal remnant of the cloaca is that adulthood is attached to the bladder dome and runs to the navel.
Clinical case. Male 28 years- old presents umbilical and periumbilical pain with hyperemia and hyperthermia; physical examination revealed hyperkeratotic area in umbilical region without of purulent material, distended abdomen, painful on palpation with a poorly circumscribed mass without abdominal wall defects. Laparotomy was performed with resection of adhering omentum, resection of the urachus and omphalectomy. He was hospitalized for three days with good performance, and the result of pathology was: ride with chronic and acute ulcerated inflammation with granulation tissue and repair, compatible with urachal sinus.
Conclusion. The abscessed urachal sinus occurs in 30% of cases. There is only one case that was located near the bladder with urachal abscess diagnosed with litho. The great risk of these abnormalities urachal in adulthood is the possibility of cancerous degeneration; it is known that these cases occur 1 x 5 million with a ratio of 4: 1 male/female and carcinomas may be tubular, papillary, mucinous or colloids and signet ring, being more frequent mucinous and better prognosis because treatment is surgical with complete resection, five-year survival is between 43 and 50%, they are radioresistant and chemo sensitivity doubtful.
