Surgical treatment of complicated diverticular disease

Objective: To inform our experience and results in the management of complicated diverticular disease.
Design: Retrospective, observational study.
Setting: Third level health care hospital.
Patients and methods: We studied retrospectively the clinical records of 32 patients with complicated diverticular disease, from November 1995 to October 1998. We analyzed the following variables: age, gender, surgical and pathological antecedents, type of surgical procedure, surgical time, volume of hemorrhage, morbidity and mortality.
Results: We studied 32 patients with an average age of 60.5 years, 18 men and 14 women.
Frequency of complication was: Fistula (50%), previous diverticulitis (25%), stenosis (6.2%), hemorrhage (9.7%), perforation (9.7%). Programmed surgery was performed in 26 patients; of these, manual anastomosis was performed in three (9.3%), in 23 patients a CH 33 circular stapler was used. Sigmoidectomy was performed in 20 patients, left hemicolectomy in six, subtotal colectomy in four, and high and low anterior resection in one, each. Six patients required emergency surgery (5 colostomies and 1 ileostomy). Surgical time in minutes was 225.62 ± 74.49, and the calculated volume of hemorrhage (ml) was of 379.68 ± 305.81. Morbidity was of 12.5%, no deaths occurred.
Conclusion: Stenotic and fistulous complications can be handled in one surgical time, without a stoma, and low morbidity or mortality. If emergency surgery is required (hemorrhage, fecal peritonitis) it is suggested to perform a stoma with distal closing of the rectum.
