Treatment of pulmonary metastases due to colo-rectal cancer

ABSTRACT

Objective: To analyze the literature regarding pulmonary metastases due to colorectal cancer, especially aimed at identifying patients liable to be treated surgically.
Setting: Third level health care hospital.
Data collection: We analyzed the most important series published in the Western literature during the last decade (21 references).
Selection of studies: We analyzed those studies presenting an analysis of treatment results and long-term survival prognostic factors.
Data extraction: It was found that up to 4% of colorectal cancer patients course with isolated pulmonary metastases and, in the absence of other diseases, are candidates for surgical resection with curative purposes, producing a long-term survival among the selected patients of 21 to 43%; no other therapeutic modality offers a cure. Not all patients benefit from the resection of pulmonary metastases, therefore, it has been attempted to identify the prognostic factors for long-term survival.
Results and conclusions: Although several factors have been identified to be associated with better survival rates in surgically treated patients, such as disease-free period, number of metastases and ACE levels, these are not perfect, hence, no patient should be discarded a priori if having an adverse factor.
