Anterior access route for post-incisional lumbar hernioplasty

ABSTRACT

Objective: To present an anterior access route to handle a difficult problem, such as multioperated post-incisional lumbar hernias.
Setting: Second level health care hospital.
Description of the cases: Report on two patients with incisional lumbar hernia, both after surgery to solve a right renal lithiasis. One woman, with five previous plasties, two with direct closure and three using mesh. The other patient, a man, with one previous plasty with direct closure. In all the plasties, recurrence occurred before 4 months. They were subjected to a new plasty with anterior access, without tension, using Stoppa’s technique (preperitoneal mesh) to avoid fibrotic and overworked on areas. In both patients, surgery was laborious and satisfactory, without trans-surgical or early complications. Patients were discharged from the hospital on the third day, and were followed-up every 3 months. At present (a year and a half for the woman and 6 months for the man) both patients have evolved well, without recurrences, and have returned to their normal activities.
Conclusion: We used an anterior access route, outside the fibrotic area, avoiding rests of previous sutures and prosthetic materials. Just as all post-incisional plasties, this is a laborious procedure that is compensated by the ease to expose the inguinal ring.
