Central mandibular xanthoma. Presentation of a case, follow-up at 2 years

ABSTRACT

The objective of this article is to report the clinical, radiographic, tomographic and histological findings about the clinical case of a female patient of 35 years of age who is referred to our center due to the presence of a behavioral lesion and clinical characteristics nonspecific in mandibular body and branch on the right side. As an initial treatment it was decided to perform a preoperative biopsy because the previous histopathological reports were not conclusive to determine the origin of the lesion, finally obtaining a histopathological result, it is determined that the lesion corresponds to a central mandible xanthoma, proceeding to perform treatment Surgical reconstruction by resection of the right side of the body and mandibular ramus and its immediate reconstruction through the placement of a reconstruction plate with condylar attachment, after the final histopathological result, and obtaining margins free of injury, proceed To perform, functional reconstructive therapy, through the elaboration of mandibular maxillofacial prosthesis, because patient does not accept surgical alternative of treatment with autologous graft (AG), due to risk and surgical morbidity associated to surgical procedure. According to current literature, central xanthomas of the jaws not associated with underlying endocrine or metabolic abnormalities are extremely rare benign bone lesions, so far, in the world literature, only less than ten cases of xanthoma have been reported in the mandible. Currently the patient has a follow-up at two year, without clinical or tomographic data of relapse of the lesion, with acceptable aesthetic and functional results.
