Caso inusual de mielitis transversa longitudinalmente extensa secundaria a Mycobacterium bovis

RESUMEN

La mielitis transversa longitudinalmente extensa (mtle) es una mielopatía de etiología tanto inflamatoria (infecciosa o autoinmune) como no inflamatoria (neoplásica o traumática). Mycobacterium bovis es una especie de micobacteria común en México, aunque infrecuente en otras partes del mundo. Se presenta el caso de un paciente de 35 años de edad con el diagnóstico reciente de meningoencefalitis tuberculosa. Durante el tratamiento su estado clínico empeoró secundario a una vasculopatía por tuberculosis (tb) y a la mtle. Tras recibir esteroides, únicamente mejoró el componente sensitivo radicular. Luego de excluir otras causas, se inició tratamiento de segunda línea para tb y después de seis meses se reevaluó al paciente mediante la escala de Rankin modificada (mrs), no se observó mejoría o evolución. La mielitis por tb es una condición devastadora que se debe considerar como una causa de mtle en regiones endémicas.
