Unusual case of longitudinally extensive transverse myelitis secondary to Mycobacterium bovis

ABSTRACT

Longitudinally extensive transverse myelitis (letm) is a myelopathy of inflammatory (infectious or autoimmune) and non-inflammatory (neoplastic and traumatic) etiology. Mycobacterium bovis, a species common in Mexico but rare in other parts of the world, was identified as the causative agent in the following case report from a 35-year-old male recently diagnosed with tuberculous meningoencephalitis, who, while being treated, his clinical condition worsened due to vasculopathy and letm. After receiving steroids, only the sensitive radicular component improved. Excluding other causes, we started second-line treatment for tuberculosis and reevaluated the patient after six months with the modified Rankin scale (mrs) without finding improvement or progression. tb myelitis is a devastating condition that should be considered as a cause of letm in endemic regions.
