La dermatitis por contacto de la piel cabelluda: artículo de revisión

RESUMEN
ANTECEDENTES: La dermatitis por contacto de la piel cabelluda
es frecuentemente subdiagnosticada. Esta se expone
recurrentemente a alérgenos y agentes irritantes debido
a la utilización de accesorios y productos para el cuidado
capilar. Sus características anatómicas le confieren cierta
resistencia al desarrollo de lesiones inflamatorias, presentándose
mínima afectación de este sitio, con mayor
compromiso en áreas adyacentes, dificultando su reconocimiento,
retrasando su diagnóstico y tratamiento.
MATERIAL Y MÉTODO: Se realizó una revisión narrativa de la
literatura, incluyendo artículos originales, revisiones sistemáticas
y series de casos relevantes sobre dermatitis por
contacto de la piel cabelluda. Se analizaron aspectos epidemiológicos,
agentes causales más frecuentes, manifestaciones
clínicas, hallazgos histopatológicos, utilidad de las
pruebas epicutáneas y enfoques terapéuticos actuales.
RESULTADOS: La revisión de la literatura mostró que la dermatitis
por contacto de la piel cabelluda se asocia principalmente
al uso de productos cosméticos capilares, siendo
los tintes para el cabello la causa más frecuente en algunos
estudios. Otros alérgenos reportados son los conservadores,
las fragancias y los metales. Clínicamente, predominan
el eritema, la descamación, el prurito, con patrones atípicos
que dificultan su diagnóstico diferencial. Las pruebas
epicutáneas son esenciales para confirmar su diagnóstico,
identificar agentes potenciales y de esta manera evitar su
contacto reduciendo su recurrencia.
CONCLUSIÓNES: La dermatitis por contacto de la piel cabelluda
representa un reto diagnóstico que requiere un elevado índice de sospecha clínica. Las pruebas epicutáneas continúan
siendo la herramienta diagnóstica de referencia. Un
abordaje integral que incluya la identificación y evitación del
agente causal, así como el uso de tratamientos antiinflamatorios,
permite un adecuado control de la enfermedad y
mejora la calidad de vida de los pacientes.
