Ratio ventilatorio en pacientes operados de cirugía cardiotorácica con y sin fracaso de la extubación

RESUMEN
Introducción: la extubación en pacientes con ventilación mecánica invasiva (VMI) es frecuente en la unidad de cuidados intensivos (UCI), pero aún presenta considerables tasas de fracaso. El ratio ventilatorio (RV), es un índice utilizado para evaluar la eficiencia ventilatoria en el síndrome de distrés respiratorio agudo (SDRA), que ha sido subutilizado para predecir fracaso de la extubación en pacientes postquirúrgicos de cirugía cardiotorácica.
Objetivo: comparar el ratio ventilatorio en pacientes con y sin fracaso de la extubación.
Material y métodos: estudio descriptivo, observacional, comparativo, transversal, unicéntrico, prospectivo y homodémico realizado en pacientes > 18 años operados de cirugía cardiotorácica; se excluyeron a pacientes con extubación fortuita y traqueostomía. Se calculó el RV con la fórmula RV = [VE medido (mL/min) × PaCO2]/(100 × peso predicho × 37.5) y se evaluaron las diferencias entre los pacientes con y sin fracaso de la extubación. Para el análisis descriptivo se utilizaron medias, medianas, frecuencias, porcentajes y valores mínimos y máximos; para comparar las medias del RV se utilizó la prueba U de Mann-Whitney y para variables categóricas chi cuadrada (χ2). Analizamos el rendimiento del RV con curva ROC y la mortalidad con curva de Kaplan-Meier y se tomó un resultado estadísticamente significativo al valor p < 0.05.
Resultados: se analizaron a 115 pacientes con mediana de edad de 57 [18-82] años. Encontramos que ser mujer (p = 0.039), tener baja estatura (p = 0.001), bajo peso predicho (p = 0.001), hipotiroidismo (p = 0.030) y dosis subóptimas de milrinona (p = 0.002), favorecen el fracaso de la extubación. También un elevado volumen tidal (p = 0.003) y elevado volumen minuto (p < 0.001) disminuyen el porcentaje de weaning exitoso. Obtuvimos diferencias en el RV entre grupos de estudio, un RV > 1.8 mostró mejor rendimiento predictivo para fracaso de la extubación frente a la p01 y el Yang y Tobin. Finalmente, la mortalidad fue mayor en pacientes con fracaso ventilatorio y RV > 1.8 (30.7%) frente a RV < 1.8 (19.6%).
Conclusión: el RV fue diferente en pacientes operados de cirugía cardiotorácica con y sin fracaso de la extubación.
