INTRODUCCIÓN

El término fast-track en cirugía ortopédica surgió en la década de 1990 como una estrategia orientada principalmente a reducir la estancia hospitalaria en artroplastía electiva de cadera y rodilla. Con el tiempo, este concepto evolucionó hacia el modelo ERAS, que integra intervenciones perioperatorias basadas en evidencia a lo largo de tres fases: preoperatoria, intraoperatoria y postoperatoria, con el objetivo de atenuar la respuesta al estrés quirúrgico, preservar la función orgánica y acelerar la recuperación funcional del paciente.
La distinción entre fast-track y ERAS no es meramente semántica. El primero se enfoca en la velocidad del alta hospitalaria como objetivo primario, mientras que el segundo prioriza la calidad de la recuperación, la seguridad del paciente y la reducción de la morbilidad perioperatoria como metas fundamentales, siendo el alta temprana una consecuencia natural de una recuperación óptima y no un fin en sí mismo.
