RESUMEN

ANTECEDENTES: La enfermedad de Gaucher es un trastorno lisosomal poco frecuente, caracterizado por citopenias, hepatoesplenomegalia, alteraciones óseas y retraso del crecimiento. El diagnóstico se confirma mediante pruebas enzimáticas y genéticas que evidencian deficiencia de β-glucocerebrosidasa y mutaciones en el gen GBA. El tratamiento de elección es la terapia de reemplazo enzimático.
CASO CLÍNICO: Paciente femenina de 7 años con antecedente de epistaxis, trombocitopenia y visceromegalia. La biopsia de médula ósea mostró células compatibles con enfermedad por depósito lisosomal. Se confirmó enfermedad de Gaucher mediante deficiencia enzimática, concentraciones elevadas de liso-Gb1 y la identificación de variantes patogénicas bialélicas en el gen GBA.
CONCLUSIONES: La enfermedad de Gaucher con frecuencia se subdiagnostica. Su reconocimiento clínico es decisivo, especialmente en pacientes pediátricos con trombocitopenia e hiperesplenismo no explicados. El diagnóstico y tratamiento temprano mejoran el pronóstico y la calidad de vida.
