Tumor neuroectodérmico primitivo renal-Sarcoma de Ewing. Revisión de la literatura y reporte de caso

RESUMEN
Descripción del caso clínico: mujer de 26 años con antecedente de
VIH y cáncer de mama triple negativo en remisión presentó lumbalgia,
fiebre y pérdida de peso. En su abordaje mediante una tomografía
contrastada se evidencio una masa renal derecha. Se realizó
una nefrectomía radical abierta y en el reporte histopatológico se
confirmó un tumor neuroectodérmico primitivo mediante inmunohistoquímica
positiva para CD99 y FLI-1. A pesar de recibir un ciclo
de quimioterapia adyuvante, la paciente presentó progresión rápida
de la enfermedad y optó por manejo paliativo, falleciendo a los 7
meses después de la cirugía.
Relevancia: los tumores neuroectodérmicos primitivos renales son
neoplasias raras y agresivas, frecuentemente confundidas con otros
tumores renales. Presentan alta recurrencia y rápida diseminación
metastásica.
Implicaciones clínicas: el manejo de los tumores neuroectodérmicos
que son una entidad poco frecuente y agresiva, requiere un enfoque
terapéutico multimodal que combine cirugía, quimioterapia y
en ocasiones radioterapia.
Conclusiones: la detección de tumores renales ha incrementado con
el uso creciente de estudios de imagen; Sin embargo, la identificación
temprana de tumores raros y agresivos sigue siendo un desafío,
lo que dificulta su manejo adecuado.
