Reincorporación de actividades y capacidad funcional en pacientes postoperados de artroplastía total de cadera con abordaje anterior versus abordaje lateral directo a 90 días

RESUMEN
Introducción: el tipo de abordaje quirúrgico
en la artroplastía total de cadera puede tener un impacto
significativo en la calidad de vida y en la reintegración de
los pacientes a su entorno cotidiano.
Material y métodos:
estudio analítico, longitudinal, observacional y prospectivo
con pacientes sometidos a artroplastía total de cadera programados
para abordaje anterior o lateral, mayores de 18
años, de ambos sexos, metabólicamente estables. Se analizó
edad, sexo, peso, talla, IMC, toxicomanías, lateralidad de la
lesión, tiempo de evolución, incorporación a actividades cotidianas
con el Cuestionario de Actividades de la Vida Diaria
versión Tecnológica (T-ADLQ) y la capacidad funcional
con la Escala de Cadera de Harris (HHS), se compararon
resultados según abordaje.
Resultados: 179 pacientes, 105
con ALD y 63 con AAD; 87 mujeres y 92 hombres, la edad
fue de 69.0 años [RIC 66.0-77.0], la cadera izquierda fue
la más afectada. En el T-ADLQ basal se tuvieron 43.7 ±
6.5 puntos (AAD) y 40.9 ± 6.4, (ALD) (p = 0.007). En el
seguimiento tuvieron 39.0 ± 6.3 puntos y 36.6 ± 6.5, respectivamente
(p = 0.020). En los dominios se tuvieron diferencias
significativas en autocuidado, hogar y viajes, tanto en
el basal como en el seguimiento. En el HHS basal, en ambos
lo más frecuente fue un resultado pobre (p = 0.231), en el
seguimiento en ambos el resultado fue bueno (p = 0.549).
Conclusión: ambos abordajes resultaron efectivos para la
funciocapacidad
funcional y la reincorporación a las actividades
de los pacientes sometidos a una artroplastía total de cadera.
