Neoplasia pseudopapilar sólida del páncreas en una paciente embarazada. Reporte de caso y revisión de literatura

RESUMEN
La neoplasia pseudopapilar sólida del páncreas es una entidad
neoplásica poco frecuente, representa del 1-2% de los tumores
de páncreas, de crecimiento lento y bajo potencial maligno. Histológicamente
presenta un patrón mixto de estructuras sólidas,
pseudopapilares y pseudoquísticas. Su origen sigue siendo
objeto de debate. Aunque clínicamente puede sospecharse
su presencia, el diagnóstico se establece por histología y se
confirma por inmunohistoquímica, con un fenotipo característico
positivo para E-cadherina. Debido a la presencia de receptores
de progesterona, estos tumores pueden verse influidos en su
crecimiento durante los periodos de altos niveles de progesterona.
Presentamos el caso de una paciente de 25 años con dolor
abdominal y sangrado uterino anormal; durante su evaluación,
se identificó una gran tumoración pancreática. Se realizó una
revisión de la literatura en la que se encontró que la influencia
hormonal juega un papel en las neoplasias pseudopapilares
sólidas del páncreas y se evaluó la evidencia del uso de bloqueadores
hormonales para el manejo prequirúrgico.
