Equipo de respuesta rápida, escala NEWS y telemetría clínica: experiencia en un Hospital de Tercer Nivel en México

RESUMEN
Introducción: el deterioro clínico intrahospitalario es frecuente y potencialmente prevenible. Los equipos de respuesta rápida (ERR) y la escala
National Early Warning Score (NEWS) buscan detectarlo de forma oportuna.
Objetivo: validar la escala NEWS y evaluar el impacto del ERR y de un sistema de telemetría clínica en un hospital de tercer nivel en México.
Material y métodos: estudio observacional retrospectivo. Cohorte piloto (marzo 2021-abril 2023, n = 81): regresión logística y curvas ROC. Cohorte completa (marzo 2021-febrero 2025) y telemetría Aidicare (2023-marzo 2025).
Resultados: NEWS fue predictor independiente de mortalidad (OR 1.215, IC95% 1.020-1.448, p = 0.029), traslado a unidad de terapia intensiva [UTI] (OR 1.294, p = 0.013), ventilación mecánica invasiva (OR 1.366, p = 0.006) y vasopresor (OR 1.193, p = 0.032); mejor AUC para ventilación mecánica (0.781; corte > 8). Causas de activación: hipotensión (41.1%) y deterioro respiratorio (26.5%); 58% se trasladó a UTIM, 23% a UTI; mortalidad 20%. Reducción relativa de 30.5% en códigos azules (RR 0.69, IC95% 0.46-1.03, p = 0.077). El sistema realizó 468,420 evaluaciones NEWS en 26,104 pacientes, con 11,198 eventos severos y un número necesario para evaluar (NNE) de 53.
Conclusiones: la integración de NEWS, telemetría clínica y ERR permitió detectar y estratificar el deterioro clínico, optimizando el balance entre beneficio y uso de recursos.
