RESUMEN

Antecedentes: las infecciones por virus de hepatitis B y C (VHB-VHC) en los individuos infectados con VIH pueden escapar al control inmunitario y generar enfermedad crónica algunas veces oculta.
Objetivo: conocer la existencia de coinfección del VIH con VHB o VHC en pacientes atendidos en un hospital privado de la Ciudad de México.
Material y método: estudio transversal realizado de octubre de 2009 a junio de 2012 que incluyó a todos los pacientes con VIH atendidos en el Servicio de Infectología del Centro Hospitalario Sanatorio Durango. Se les solicitó: transaminasas, serología para virus de hepatitis A (VHA), B (VHB) y C (VHC), cargas virales de hepatitis B, C y VIH y recuento de linfocitos CD4. El análisis se realizó utilizando el programa estadístico SPSS 13.0.
Resultados: se incluyeron 41 pacientes, 90% (n = 37) eran hombres y 82% (n = 34) recibía tratamiento antirretroviral altamente activo al inicio de la evaluación; 32 acudieron a realizarse los estudios, 27 se realizaron estudio de carga viral de VHB, VHC y VIH y 29 serología de hepatitis viral; se encontró que todos tenían IgG contra VHA. Nueve pacientes mostraron anticuerpos contra antígeno de superficie (31%) y uno tuvo anticuerpos anti-VHC (3.4%). Las cargas virales mostraron sólo un paciente con viremia detectable de VHB y tres con carga viral detectable de VHC.
Conclusiones: los pacientes con VIH/SIDA, aun cuando están en tratamiento antirretroviral y tienen adecuado recuento de linfocitos CD4 y carga viral de VIH indetectable, pueden tener coinfección activa e incluso oculta por virus de hepatitis B, C o ambos, por lo que este estudio demuestra la importancia de vigilar esa coinfección.
