Double abdomino-thoracic and pelvic-abdominal impalement

ABSTRACT

Introduction. Impalement term is used to describe injuries accidentally produced in which an object passes through the body of a person. We present a case of double accidental impalement with surgical treatment and patient survival.
Clinical case. Male patient aged 34 years-old was admitted to the emergency department by presenting penetrating wounds pelvis, abdomen and chest 30 minutes of evolution caused by two rods one inch in diameter. The vital signs of income were: heart rate (HR) 120 rpm, blood pressure (BP) 80/60 mmHg, respiration rate (RR) 24 bpm, temperature 98.6 ºF. Physical examination identifies hypoventilation of the left lung, and subcutaneous emphysema; the abdomen has a rod which enters through the right flank until the mid axillary line emerging level 5th left intercostal space. On the right thigh other rod is observed with same trajectory at 20 cm from the first to hint at the axillary skin midline of the left flank. Laboratory studies showed: hemoglobin (Hb) 11.0 g/dL, leukocytosis 12,600/ mm. Abdominal exploratory laparotomy performed plus left thoracotomy with the following findings: the first rod penetrates right flank, drills left hepatic lobe, left extrapulmonary and penetrates in left lung field. The second rod has trajectory approximately 20 cm below the first rod via retroperitoneal, proceeded to the removal of both.
Conclusion. This patient did not injury of vital organs and survived despite the impressive thing about his wounds.
