Recurrent infection of the parotidomaseterine space of uncertain etiology

ABSTRACT

Introduction: the calcification of the stylohyoid ligament is a dystrophic condition where insoluble mineral salts are deposited in the soft tissues; the surgeon's early diagnosis using advanced imaging techniques might be the best guarantee to select the right clinical protocol. Objective: to describe the importance of radiographic diagnosis in the management of recurrent infection of the parotidomaseterine space associated with calcification of the stylohyoid ligament. Presentation of case: a 9-year-old female patient started symptoms with an 7 cm diameter increase in volume in the left parotidomaseteric region, echosonography and magnetic resonance imaging were evaluated, evidencing increased thickness of the parenchyma of the ipsilateral parotid gland; computed tomography data was analyzed, observing a hyperdense image with longitudinal course in apparent relation to the left parotid duct diagnosing calcification of the stylohyoid ligament and its association with the recurring infection. Results: after permeabilization of the affected parotid duct, there was a notable clinical improvement, absence of signs of phlogosis and unpleasant symptoms. Daily controls were carried out for 5 days, evidencing facial symmetry improvement, no pain on palpation, after three months control presented a slight increase in volume to physical efforts. Conclusion: calcification of the stylohyoid ligament is reported as a radiographic finding in 23% of cases, the predisposing factors are elderly patients, previous trauma to the neck and the parotidomaseteric region, although it is a pathology of unknown etiology, it can trigger clinical manifestations similar to infectious odontogenic, glandular and lymph node pathologies, for which its importance is raised in terms of its image-guided diagnosis.
