Syndrome of irreversible lithium effectuated neurotoxicity (SILENT) and bipolar disorder: a case report

ABSTRACT

Introduction: syndrome of Irreversible Lithium Effectuated Neurotoxicity's main manifestations are vermian cerebellar disorders, persistent extrapyramidal syndrome, brainstem dysfunction and dementia. We aim to communicate this rare complication of lithium treatment, on purpose of a patient with type 1 bipolar disorder. Clinical case: female patient, 47 years old, previously diagnosed with type 1 bipolar disorder, treated with unknown dosage of lithium. Admitted at Internal Medicine service because of a moderate-severe lithium intoxication, persisting with neurological and psychiatric sequelae for over two months long since her admission. Laboratory, radiological and cognitive studies were performed, showing frontal anatomical and functional changes as well as cognitive impairment. Conclusion: we highlight the importance of a correct therapeutic control and close evaluation of lithium's serological levels, in order to prevent permanent neurological sequelae, causing significant dysfunction.
