Peripheral ossifying fibroma.

ABSTRACT

Gingival enlargements are common, presenting as lesions such as fibrous hyperplasia, pyogenic granuloma, peripheral giant cell granuloma, and peripheral ossifying fibroma (POF). POF is a localized reactive lesion that originates in the gingiva, characterized by a single, firm, asymptomatic nodule with slow growth. Its color may range from red to purple, and it can bleed with a smooth or ulcerated surface. It typically develops from the interdental papilla, with a pedunculated or sessile base, and measures less than 2 cm. The objective of this article is to report a clinical case of POF, describing its clinical characteristics, treatment, and histopathological findings. The patient is a 31-year-old woman, whose intraoral examination revealed an oval, pedunculated, erythematous gingival enlargement, non-painful, firm to palpation, and approximately 18 × 13 mm, located in the interdental gingiva between the upper central incisors. The treatment consisted of a full-thickness excisional biopsy, which was sent for histopathological study. The surgical procedure involved raising a full-thickness flap to ensure the complete removal of the root calculus, eliminating any potential sources of irritation that could cause recurrence. Given the clinical variability of gingival lesions with similar appearance, histopathological analysis is always required to confirm the diagnosis of peripheral ossifying fibroma.
