Distinguishing the indistinguishable: diabetic nephropathy in non-diabetic patients

ABSTRACT

Known as diabetic nephropathy in non-diabetic patients, this is the name given to a group of glomerulopathies that are histologically indistinguishable from diabetic nephropathy that occurs in patients without diabetes (with or without altered glucose metabolism). Histologically, it is characterized by mesangial expansion, nodular sclerosis in more than 95% of cases, interstitial fibrosis and tubular atrophy in 25% of cases, and hyalinosis with arteriolar sclerosis in up to 86% of cases. The objective of this research is to report a series of cases with diabetic nephropathy in patients without diabetes and to conduct a review of the topic. Three cases of patients without a diagnosis of diabetes are presented whose histopathological findings in renal biopsies report diabetic nodular nephropathy, its evolution, treatment and a review of the topic. This entity is increasingly gaining strength as a histopathological disease to be considered in patients with nephrotic syndrome. In our patients, the function remained impaired but stable and there was a good response to pharmacological treatment. Their risk history coincides with those reported in the literature, although it is vitally important to adequately determine the markers of metabolic function.
