Corticotomy for periodontally accelerated osteogenic orthodontics and gingival recontouring: case report

ABSTRACT

Traditional orthodontic treatment represents pain and prolonged time for the patient, which causes some patients to refuse or interrupt treatment. Skeletal and dental Angle class II patients pose a greater challenge when trying to achieve facial harmony without undergoing orthognathic surgery. Periodontally accelerated osteogenic orthodontics (PAOO) through corticotomies is an alternative that uses as a biological basis the regional acceleration phenomenon (RAP), a tissue reaction to different harmful stimuli such as infections, trauma, fractures and surgical interventions. It is characterized by an acceleration of normal tissue processes involving soft and hard tissues. On the other hand, it improves the different stages of normal physiological healing, which occurs 2 to 10 times faster, resulting in greater orthodontic movement, reducing pain, root resorption, loss of bone structure and treatment time. RAP begins a few days after trauma, with high peaks in the first two to four months and lasts up to 24 months. The use of piezoelectric to perform this treatment reduces tissue damage, produces osteopenia that increases bone mineral content. The objective of this work is to detail the technique used in a patient with these characteristics accompanied by a gummy smile and the benefits of using corticotomies for rapid tooth movement.
