Multiorgan procurement in a pediatric patient with a pineal gland tumor

ABSTRACT
Introduction: transplantation is the treatment that can be definitive for many serious diseases. In general, adults are the main organ donors, even for pediatric patients. The indications for donation have been established for several decades. In some guidelines, cancer is considered a contraindication; however, there are studies where successful cases of donation have been documented, especially from patients with central nervous system (CNS) tumours.
Clinical case: we present an adolescent diagnosed with germ cell tumor of the pineal gland, who began with clinical signs of severe cranial hypertension. After diagnosis, surgery was performed to remove the tumor, but in the postoperative period there were complications leading to brain death. The parents authorized multi-organ donation after a conversation with a multidisciplinary team. The heart, liver, kidneys and corneas were obtained.
Conclusion: given the low frequency of donation from pediatric patients, it seems necessary to have an in-depth understanding of the donation criteria, including pathologies such as cancer, to increase transplant opportunities.
