New genioplasty technique that preserves the mentolabial fold: technical note

ABSTRACT
Chin surgery is one of the surgical procedures that has the greatest impact on facial harmony. Since it involves the lower third of the face, it is crucial to consider the mentolabial fold as a key component in achieving both aesthetic and functional outcomes. The objective of this article is to propose a new mentoplasty technique for patients presenting with an adequate mentolabial fold but requiring correction of vertical macrogenia and horizontal microgenia, by excluding the mentolabial fold and/or the point of maximum mandibular concavity (Point B) from the osteotomy, and modifying only the projection of the Pogonion (Pg) through counterclockwise rotation. The trapezoidal-shaped osteotomy is performed via a minimally invasive approach, where the vertical cuts are camouflaged by the depressor anguli oris and depressor labii inferioris muscles to prevent notching. This technique aims to achieve aesthetic improvements by enhancing the projection of the Pg point, creating balance and harmony among the facial thirds, while preserving the position of Point B.
