Glandular odontogenic cyst: clinical case report and literature review

ABSTRACT

Introduction: the glandular odontogenic cyst (GOC) is a benign developmental lesion with aggressive behavior, arising from the dental lamina and characterized by glandular differentiation in its epithelial lining. It represents only 0.2% of all odontogenic cysts. Objective: to report a clinical case of GOC with an unusual maxillary location and highlight the value of immunohistochemistry in its diagnostic confirmation, complemented by a literature review on its diagnostic and therapeutic implications. Case presentation: a 22-year-old male from Xalapa, Veracruz, presented with a six-month history of an asymptomatic swelling in the left buccal region. Intraoral examination revealed expansion of the vestibular cortical plate and buccal sulcus of the maxilla. CT imaging showed a 30 × 30 mm oval lesion with cortical perforation and communication with the maxillary sinus, in close proximity to the infraorbital nerve. An excisional biopsy was performed, and histopathology confirmed the diagnosis of GOC. The postoperative course was favorable, without complications. Conclusion: GOC requires specific diagnostic criteria and long-term clinical follow-up because of its recurrence potential.
