Efficacy of tranexamic acid in reducing perioperative blood loss and transfusion rates in multilevel spinal surgery: a retrospective cohort study

ABSTRACT
Introduction: complex spinal surgeries are often associated with significant blood loss, which can increase morbidity, the need for transfusions, and prolonged hospital stays. Tranexamic acid (ATX) is an antifibrinolytic agent used in various surgical specialties to reduce bleeding.
Objective: to evaluate the efficacy of tranexamic acid in reducing intraoperative and postoperative bleeding and the need for transfusions in patients undergoing thoracolumbar or lumbosacral spine surgery with multilevel instrumentation.
Material and methods: a retrospective cohort study was conducted in 67 patients who underwent surgery between March 2022 and March 2024. Two groups were compared: one received ATX (n = 23) and the other did not (n = 44). Clinical, transfusion, and hematological variables were analyzed.
Results: no statistically significant differences were found in intraoperative blood loss volume or transfusion rates between the groups. However, the group treated with ATX showed slightly higher postoperative hemoglobin and hematocrit levels, with less variability.
Conclusion: although the use of ATX did not show a significant reduction in blood loss or transfusions, it could promote greater postoperative hemodynamic stability. Prospective, multicenter studies with larger sample sizes are needed to confirm these findings.
