Modification to the technique of anterolateral thigh flaps improving surgical times

ABSTRACT
The anterolateral thigh flap (ALT) is widely used in reconstructive surgery, although its dissection may be prolonged due to anatomical variations and Doppler use. The purpose of this treatment is to reduce surgical time and design the skin island according to the defect size required. We conducted a retrospective study of five patients operated between 2023 and 2025, who underwent reconstruction with ALT flaps using a modified technique: lateral incision without Doppler marking, direct dissection towards the pedicle and transoperative design of the cutaneous island. Five men between the ages of 23 and 54 years-old were included. No complications were reported. In three cases, grafting was required in the donor area, while in two cases, primary closure was achieved. The technique showed a reduction in operating times and facilitated flap design. We conclude that the modified ALT flap technique shortens surgical times without compromising safety. The preoperative Doppler is not essential, and the cutaneous island can be designed individually during surgery.
