Nasolabial flap: an alternative for the reconstruction of large oronasal fistulas in patients with cleft lip and palate

ABSTRACT

Cleft lip and palate (CLP) is one of the most common congenital malformations, with an incidence in Chile of 1.7 per 1,000 newborns, double the global rate. Its etiology is multifactorial, and its management requires a multidisciplinary approach. Despite advances in reconstructive surgery, oronasal fistulas (ONF) remain one of the most common complications, particularly affecting patients with bilateral clefts. The treatment of these fistulas depends on their size, location, and severity of existing scarring. Alveolar fistulas represent a surgical challenge due to the presence of fibrous tissue and the proximity of developing teeth. Although various techniques exist for their closure, there is no consensus on the ideal method, and the approach to adult patients with untreated clefts is poorly documented in the literature. This article presents the nasolabial flap as a viable alternative for the closure of large ONF in adults. Its planning, execution, and postoperative follow-up are described, highlighting its usefulness in patients with severe defects and a history of failed treatments. This technique, although not a standard of care, expands the therapeutic options for these complex cases.
