Preseptal cellulitis and cerebritis as complications of pansinusitis

ABSTRACT
Introduction: pansinusitis can lead to orbital and intracranial
complications, which are infrequent but can be serious.
Case presentation: a previously healthy 13-year-old male
adolescent presented with persistent frontal headache
and three days of fever, followed by left eyelid edema and
erythema. Imaging studies revealed preseptal cellulitis
secondary to pansinusitis. On the fifth day of hospitalization,
despite initial antibiotic treatment, he experienced a focal
motor seizure and altered mental status. Magnetic resonance
imaging identified left frontal cerebritis, and broad-spectrum
antimicrobial and anticonvulsant therapy was initiated.
Endoscopic sinus surgery was also performed. He completed
four weeks of antibiotic therapy with good clinical progress
and no neurological sequelae.
Conclusions: this case
highlights the importance of timely detection of sinusitis
complications and the need for a multidisciplinary approach
to improve the prognosis for these patients.
