ABSTRACT

The first dorsal metacarpal artery flap was described in 1960 by Kuhn and Holevitch, who used the skin fon the back of the proximal phalanx of the index finger as a donor site for fasciocutaneous flaps. Steve Rocha described indications for the kite flap, including coverage of defects on the dorsal thumb and coverage of thumb amputations at the metacarpophalangeal joint level. The pedicle consists of the first dorsal metacarpal artery; typically a terminal branch of the radial artery. This flap has a consistent vascular pedicle; anatomical variations exist among individuals in its terminal branches and course across the first web space; however, it is present in 100% of individuals. We present the case of a 43-year-old man with a crush injury in the right thumb. Initial management included wound cleaning and closure by a private physician. Subsequently, failing to improve, with increased swelling and purulent discharge, he sought further care. A variant of the kite flap was performed with good results.
