Primary hyperparathyroidism due to giant ectopic parathyroid adenoma in the upper mediastinum

ABSTRACT

Primary hyperparathyroidism is a common endocrine disorder and its main cause is a parathyroid adenoma in 80-85% of cases. Up to 16% may have an ectopic location. A giant parathyroid adenoma is a rare condition that is defined as one that weighs more than 3.5g or measures more than 3cm. The only curative treatment is resection of the adenoma, it is necessary to locate it with imaging studies to carry out adequate surgical planning. Here we present the case of a 50-year-old patient with primary hyperparathyroidism caused by a giant ectopic hyperparathyroid adenoma. The lesion was located in the upper mediastinum and was resected by right thoracotomy, achieving complete healing with normalization of calcium and parathormone levels, with a short hospital stay. The choice of the best approach will have to be made based on the demographic characteristics of the patient, the location and size of the adenoma, as well as the available hospital resources.
