ABSTRACT

Onychomadesis is the complete and painless separation of the nail plate from the proximal nail fold. It has been described as a late complication of hand, foot, and mouth disease. A series of cases seen in the outpatient clinic of a private medical unit are presented.
materials and methods. Cases prior to 2022 and those following 2025 were analyzed. Patient records with a history of hand, foot, and mouth disease were reviewed, including variables such as presentation, age, sex, management, and time elapsed.
results. Thirty-eight cases of hand, foot, and mouth disease were seen between August 2022 and August 2025, predominantly in males. Eight cases of onychomadesis were also observed, five (62%) of which were in females. Three of the cases presented with the initial infection, and the rest with the nail problem. In 2023, six cases of hand, foot, and mouth disease were treated; in 2024, four cases; and in 2025, three cases. Onychomadesis was not identified in any of these cases.
conclusions. Onychomadesis is self-limiting, with the damaged nail detaching and a new nail growing in its place. There is likely a relationship with certain serotypes of the virus that predispose to onychomadesis. Family members should be informed of this possible complication in cases of hand, foot, and mouth disease.
