JAK inhibitors as treatment for moderate-to-severe atopic dermatitis: literature review

ABSTRACT

This review explores the immunopathogenesis of atopic dermatitis (AD) and the emergence of Janus kinase inhibitors (JAKi) as a cornerstone of modern therapy. As a class of drugs targeting the JAK/STAT signaling pathway, JAKi address the underlying dysregulation responsible for persistent inflammation and pruritus in AD. Their mechanism of action involves the reversible, non-covalent inhibition of JAK enzymes by occupying the catalytic ATP-binding site within the JH1 domain. Current evidence highlights the remarkable efficacy of both topical and oral JAKi in treating moderate-to-severe AD. Furthermore, ongoing research into their long-term safety profiles and molecular specificity is driving a shift toward personalized medicine, allowing for treatments tailored to a patient’s unique genetic and molecular landscape. This review aims to evaluate the clinical utility, mechanism of action, and safety of topical and systemic JAK inhibitors in the management of moderate-to-severe atopic dermatitis.
