Acute fatty liver of pregnancy and renal replacement therapy in a critically ill patient. A case report

ABSTRACT

Acute fatty liver of pregnancy (AFLP) is a rare but potentially fatal complication. It typically presents in the third trimester, although it can occur in the second trimester or postpartum. It causes severe hepatic dysfunction associated with renal impairment and prolonged coagulation times. The case of a 30-year-old primigravid patient with a 33-week twin pregnancy is described. She presented with jaundice, twin fetal demise, and the need for an urgent cesarean section. She evolved with acute hepatic and renal failure, thrombocytopenia, and anemia. She required continuous renal replacement therapy (CRRT), antimicrobial support, and transfusions. After several days in the ICU, she showed progressive recovery of liver and kidney function. She was discharged with outpatient follow-up and complete recovery.
