ABSTRACT

Purple Urine Bag Syndrome (PUBS) is an uncommon but clinically relevant condition observed mainly in patients with long-term indwelling urinary catheters. Although its striking purple discoloration may initially appear alarming, early recognition facilitates the prompt diagnosis of urinary tract infection and the identification of contributing factors such as prolonged catheterization, constipation, dehydration, and urinary bacterial colonization. The present case highlights the underlying pathophysiology involving tryptophan metabolism and the production of indigo and indirubin pigments by bacteria with sulfatase and phosphatase activity. In the emergency department, recognizing this syndrome supports a comprehensive approach that includes catheter replacement, urine culture collection, timely initiation of appropriate antimicrobial therapy, and correction of predisposing factors, thereby reducing the risk of severe complications such as sepsis. Rather than being merely an unusual finding, PUBS should be considered an important clinical sign that warrants systematic evaluation and prompt management.
