ABSTRACT
Introduction: during laparoscopic cholecystectomy,
the need for scope interchange with mini-instruments
constitutes an operative challenge that may extend the
duration of the procedure.
Objective: to compare operative
time between suture ligation and metal clips during
laparoscopic cholecystectomy.
Material and methods:
this analytical, comparative, retrospective, cross-sectional
study included 331 patients undergoing laparoscopic
cholecystectomy for acute or chronic cholecystitis.
Categorical variables were analyzed using χ
2 or Fisher’s
exact test, and continuous variables with the Mann-Whitney
U test.
Results: patients were divided into two groups:
metal clip ligation (n = 165) and non-absorbable suture
ligation (n = 166). Significant differences were observed in
antibiotic use (88.5% vs 24.7%, p ‹ 0.001), postoperative
drain placement (23.6% vs 10.8%, p = 0.002), and hospital
stay (median 2 vs 1 day, p ‹ 0.000, 95%CI 0.74-1.41).
No significant difference was found in operative time
(median 65 vs. 60 minutes, p = 0.138).
Conclusion: suture
ligation is a safe and effective alternative to metal clips,
with similar operative times and no increase in short-term
complications.
