ABSTRACT

Cecal diverticulum is a rare condition that may clinically mimic acute appendicitis, representing a diagnostic challenge in patients presenting with acute abdomen. We report the case of a 45-year-old woman who presented to the emergency department with right lower quadrant abdominal pain associated with nausea, vomiting, and fever. Physical examination revealed signs of peritoneal irritation, and laboratory studies showed leukocytosis. Abdominal computed tomography suggested acute appendicitis, and an open appendectomy was performed. During surgery, a complicated cecal diverticulum mimicking appendicitis was identified, and diverticulectomy was carried out, with favorable postoperative outcome. Cecal diverticulum should be considered in the differential diagnosis of right lower quadrant pain, as intraoperative diagnosis is common and timely surgical management prevents complications.
