Surgical treatment of the ulnar deformities in multiple osteochondromatosis

ABSTRACT

Main objective. Determine the ulnar surgery outcome in patients with MO. Material and methods. We reviewed performed in patients with MO between 1990 to 2002. We evaluated: gender, age at the time of surgery, range of motion, radial articular angle, ulnar translocation of the carpus. We divided patients in: Group 1, patients with exostosis resection; Group 2, with ulnar lengthening. Results. Nineteen patients, 4 bilateral, 11 (58%) boys and 8 (42%) girls. Thirteen forearms in Group 1, 10 forearms of Group 2. The age of surgery in Group 1 was of 10 years and 12 years in Group 2. The ulnar length discrepancy was: preoperatively –6 mm in Group 1 and –10 mm in the Group 2 and of –4 mm in Group 1 and 2 mm in Group 2 postoperatively. The Fogel’s radial articular angle was 40° in preoperative period and 31° in postoperative period this in the Group 2. Conclusions. We recommend: 1.- Osteochondroma excision at same time of ulnar lengthening; 2.- Ulnar lengthening until discrepancy is +2 mm because of forearm bones growing; 3.- Protect the forearm with a plaster cast to avoid a fracture, angulation or deformity, after removal of the external fixator for two weeks; 4.- Ulnar lengthening when: a) The Fogel´s angle is ›30°; b) Ulnar translocation of the carpus exists; c) Ulnar length discrepancy ± 10 mm; d) Radial head subluxations or dislocation exist, with pain and pronosupination disability.
