Queratoplastía penetrante con donadores mayores de 50 años

ABSTRACT

Purpose: To describe the experience of the Asociación para Evitar la Ceguera en México with the use of donated grafts from donors 50 years and older and to correlate transparency and opaqueness with age of the donated graft.
Material and methods: Clinical records between 1998 and 2000 of patients submitted to penetrating keratoplasty with corneal graft of donors with an age equal or major to 50 years were checked. The grafts had to have a minimal count of 2000 endothelial cells/mm and minimal follow-up of 5 years. The demographic characteristics, preoperative diagnosis, donating bank, age of the donor, time of death to capture the corneal graft, time of conservation and condition of the graft were obtained.
Results: 406 patient were included; 208 women and 198 men with 431 grafts. The average age of the recipients was 40.83 years, 83.29% of the cases were operated by surgeons in training. The most frequent preoperative diagnosis was keratoconus.
The average age of the donor was: 79.83 years, the average time between death and corneal graft procurement was: 6.04 hrs and the average time between death and surgery was: 11.14 days. 58.70%. Of the grafts at the end of the follow-up were transparent. The major percentage of rejection of the corneal graft appeared in grafts of donors 71 years or older.
Conclusions: In the final follow-up we do not find correlation of opaqueness of the corneal button with the experience of the surgeon. The factors that were correlated with the opaqueness of the corneal button were the preoperative diagnosis of previous rejection and age of the button 70-year-old donor or more.
