Laparoscopic cholecystectomy with an umbilical port (CL1P). Evolution of a surgical technique. The first-case presentation (1997)

ABSTRACT
This work is a brief description of the evolution in laparoscopic surgery that has had our surgical group from 1992 to 1997. The intention of this work is to present our first case cholecystectomy performed with a single umbilical port and the technical difficulties that we had at the beginning of this surgical technique, that at the moment constitutes ours first election in gallbladder pathology. A female patient of 42 years old with diagnosis of lithiasic chronic cholecystitis was operated of laparoscopic cholecystectomy with an 11 mm single umbilical port, using laparoscope with working channel of 6 mm and laparoscopic instruments of 5 mm by 43 cm in length: scissors, clamp of Maryland, multipurpose instrument (needleholder, irrigation-suction function, knotpusher, hook). The surgery was performed in 85 minutes, without complications, with greater surgical time than the average in traditional laparoscopic surgery, that was attributed to the learning curve that implies the use of laparoscope utilized as working port. Although a single case with this new technique does not have statistic relevance, the clinical success allows to encourage to its later development and improvement.
