Acuscopic cholecystectomy with 10 mm, 5 mm ports and percutaneous reins. Experience and description of the technique

ABSTRACT
Objective: To present and alternative to the percutaneous reins technique in which, aiming at causing less trauma and an adequate exposure, we substituted the laparoscopic needles, using instead 0.4 mm polyamide sutures as third port and as material to ligate the cystic conduct and the cystic artery.Setting: Regional Hospital “General Ignacio Zaragoza”.
Design: Longitudinal, prospective, and descriptive study.
Statistical analysis: Percentages as summary measure for qualitative variables.
Material and methods: Thirty patients with vesicular litiasis programed electively for acuscopic cholecystectomy. The analyzed variables were age, gender, surgical time, in-hospital stay, complications, frequency of conversion to three-port surgery or to conventional surgery, postoperative pain, cosmetic result, cost.
Results: We studied 30 patients, 22 women and 8 men, average age was of 44.3 years, surgical time was of 53 min, post-surgery inhospital stay was of 2.5 days, no conversion to three-port surgery had to be made, 25 patients referred absence of pain at 24 h post-surgery, and the cosmetic result was better.
Conclusion: The proposed modification has the advantage of saving the third port and because of the low cost of the polyamide material it is easily reproducible in any hospital having laparoscopic equipment.
