Ultrasound and clinical evaluation of neutropenic colitis

ABSTRACT

Introduction: The neutropenic colitis it’s a damage to the intestinal mucosa mainly the terminal ileum and cecum that its characterize by abdominal pain, abdominal distention, fever, vomiting and sometimes by constipation or diarrhea as well as neutropenia induced by chemotherapy in patients whit leukemia.
Patients and methods: Between April 2007 and April 2008 we performed a retrospective analysis of pediatric patients whith the diagnosis of neutropenic colitis that were introduce to the pediatric surgery service recovering type of leukemia, clinical presentation, age, sex, neutrophil count and thickness of intestinal mucosa at the cecum, ascending colon, transverse colon and esplenic angle. The neutrophil count and the thickness of the intestinal mucosa were recover at the first, third, sixth and ninth day.
Results: We recover 28 patients, 22 (79%) were ALL and 6 (21%). Both groups whit sever neutropenia at presentation and recovery between the third and sixth day for ALL and the ninth day for the AML group. The cecum and ascending colon were the most affected sites in both groups whit a much longer recovery time for the AML patients for the neutrophil count and thickness of intestinal mucosa.
Discussions: The prevalence of ALL whits AML is similar to other reports however we notice a much longer recovery time in neutrophil count and thickness of intestinal mucosa in patient’s with AML as well as poorer prognosis.
