ABSTRACT

Introduction. Irritable bowel syndrome (IBS) has a high prevalence in different physiopatogenic mechanisms and small intestinal bacterial overgrowth (SIBO) has been implicated in this condition. Lactulose breath test (LBT) is a non invasive method for SIBO diagnosis. Aim. To study the frequency of SIBO in patients with IBS symptoms using a LBT. Material and methods. Restrospective and descriptive study from August 2010 to September 2011 was conducted. We included 39 consecutive patients, referred to the Digestive Disease and Obesity Clinic in Medica Sur Clinic & Foundation in Mexico City, with IBS diagnosis (Rome III criteria). We evaluate demographic and clinical variables such as: bloating, pain, diarrhea, constipation and mixed pattern. LBT was made to all patients to determine the presence of SIBO. Results. Thirty nine patients evaluated for IBS, where the LBT was positive in 33 (84%) and negative in 6 (16%) patients. Twenty nine (75%) were female and ten (25%) men. The mean age was 46 ± 13 years. 34 patients (87.5%) had history of bloating, 20 (51.3%) pain, 18 (46.2%) diarrhea, 5 (12.8%) constipation and 4 (10.3%) had a mixed pattern. Conclusions. In this study 84% of patients with IBS had a positive LBT for diagnosis of SIBO. The most prevalent symptoms were bloating, pain and diarrhea. These results suggest that SIBO could be implicated in the pathogenesis of IBS.
