Bile duct exploration in a patient undergoing laparoscopic cholecystectomy. A case report

ABSTRACT
In high-risk patients, with cholangitis, or septicemia in case of common bile duct stones, preoperative endoscopic retrograde cholangiopancreatography is indicated. However, in the majority of elective patients, with common bile duct stone, it could be performed during the first procedure. The objective, is to show the current management of choledocholithiasis when residual choledocholitiasis its documentated in a patient without history of jaundice, pancreatitis, elevated liver function tests. We present the case a female patient of 85 years old, admitted at emergency room because abdominal pain and acute cholecystitis, a laparoscopic cholecistectomy was performed, with evidence of common bile duct dilatation. A transcistic tube was collocated in the cholecystectomy, but the medical conditions of the patient were no optimal to realize a transoperatory cholangiogram. Control cholangiogram within the first three weeks of first surgery demonstrated 3 stones at the extrahepatic bile duct. Then the patient was admitted at operating room to realize laparoscopic common bile duct exploration, 3 stones were obtained and primary close was performed, the patient was discharged 6 days later without complications.REFERENCES
